Availability of opioid pain relief is an issue here in Thailand. My understanding is you can only get it as a hospital inpatient, and even then it's reluctantly prescribed. Dying at home, pain free isn't an option for people with painful terminal illnesses.

Correct, but even then there is extensive scrutiny both internally and on an external (authorites) on the doctors prescibing opioids according to a friend of mine who is a DDS I had surgery some years back and was given morphine in the hospital, but not sent home with any opioids, i asked was i getting any, surgeon said come back in 10 days we will reassess any pain levels and we will talk about it, pretty much saying your not getting them unless you really need them

Just to show my ignorance in this topic, may I ask if there's a difference between painkillers and opioids? Are opioids just 'stronger' versions of painkillers and therefore more likely to be addictive? Could taking lots of 'ordinary' painkillers do the trick?

IVs need to be administered by a nurse or doctor though if you can get one privately the hospital will allow IV morphine. Fortunately I have never had to use it as Gavapentin and tramadol handle most of my pain problems.

They're all related to opium/morphine and can be highly addictive. (Of course, if you're terminally ill this shouldn't be an issue.) However, as a class they are very potent. Given the right dose you could have your leg cut off and not feel any pain (at the time). Even if you swallowed a barrel load of aspirin or paracetamol you wouldn't get that level of pain suppression (and would probably kill yourself trying).
